Social Prescribing Link Workers collaborate across health and care systems, focusing on populations with the greatest needs and those at higher risk of health inequalities. They also work with partners to identify service gaps and help ensure community support is both accessible and sustainable.

We have two Social Prescribing Link Workers who support adults and one dedicated to supporting children and young people.
