Registering as a patient

The doctors welcome new patients who live within our practice area.  As it often takes some time for records to be forwarded from your former practice, all newly registered patients will be asked to complete a health questionnaire.

If you wish to register at Mount Pleasant Health Centre you will need to come in to the Health Centre to complete our registration forms with proof of identification i.e Passport or Driving Licence and proof of address, if you do not have these available we are still able to register you but providing these will help us to verify your details and avoid any delay in the registration process.

Medical treatment is available from the date of registration.  Please contact reception for further information.
