How We Use Your Information To Plan Your Care

At our practice, we want to make sure every patient gets the right care at the right time. To do this, we regularly review and group patient health information so we can better understand who may need more support.

This helps us look after patients safely, prevent illness where possible, and make the best use of clinical time and resources.


What is Data Segmentation?

Data segmentation means grouping patients based on their health needs or level of risk.

For example, we may consider:

  • Ongoing health conditions (such as diabetes, or heart disease)
  • Recent test results or hospital admissions
  • How often you use healthcare services
  • Age or other factors that may affect your health

This helps us identify:

  • Patients who may need urgent care
  • Patients who would benefit from closer monitoring
  • Patients who are stable and can continue with routine care

What is RAG rating?

We use a simple system called RAG rating to help prioritise care:

  • Red – Patients who may need urgent attention or closer support
  • Amber – Patients who may need monitoring or follow-up
  • Green – Patients who are currently stable and require routine care

RAG rating is not a label about you as a person. It is a tool to help us organise care and keep patients safe.


Why do we use this system?

Using data in this way helps us to:

  • Identify patients who may be at risk of becoming unwell
  • Offer support earlier, before problems become more serious
  • Make sure urgent cases are seen quickly
  • Provide more personalised and proactive care

How do we use your data?

We only use information from your medical record, such as:

  • Appointments and visits
  • Test results
  • Diagnosis and treatments

Your information is:

  • Kept secure and confidential
  • Only accessed by authorised healthcare professionals
  • Used to improve your care and support your health

Will this affect my care?

No. This system does not change the care you receive, it helps us organise and prioritise care more effectively.

If you are identified as needing more support, we may:

  • Invite you for a review
  • Offer additional monitoring
  • Contact you to discuss your health
