Changing Your Contact Details
To update details, such as your address, contact number, or email address, choose from one of the following methods:
- in your NHS account (through the NHS app or NHS website)
- in your SystmOnline account
- fill out an eConsult online
- by visiting the practice between 8:30am to 6pm, Monday to Friday
NHS App
You can update your contact details using the NHS App.
To update your address or other details:
- Open the NHS app
- Tap More in the top-right corner
- Tap Account and Settings
- Tap Manage NHS Account
- Scroll down and tap Contact Details
- Tap Change Email Address or Change Phone Number
- Choose the details you want to update and tap Continue

Change of Address
You can change your address using any of the methods listed at the top of the page. To view our catchment area, please visit: https://www.mountpleasanthealthcentre.co.uk/out-of-area-registration/
Staying in our catchment area:
- Patients who are living within our catchment area will receive full rights to access our services.
Moving outside our catchment area
- If your new address has moved outside of our catchment area, then you might be registered as an ‘Out of Area’ patient.
- Patients who are registered as out of area will not be able to access the same services as patients living within our catchment area, such as home visit appointments.

Changing your Phone Number and/or Email Address
You may be asked to provide identification before we can update your phone number or email address.
Changing Your Name
You may need to show us proof that you have changed your name. Acceptable documents include:
- marriage certificate
- civil partnership certificate
- deed poll
- birth certificate
