Social prescribing connect people to support in their local community, from wellbeing groups and activities to practical help and emotional support.

What are Social Prescribing Link Workers?
Social prescribing is especially effective for individuals experiencing low-level mental health needs, who feel lonely or isolated, with long-term conditions and complex social needs.
Social Prescribing Link Workers collaborate across health and care systems, focusing on populations with the greatest needs and those at higher risk of health inequalities. They also work with partners to identify service gaps and help ensure community support is both accessible and sustainable.

How do they support patients?
Social prescribing link workers provide guidance and connect people to non-clinical support for a wide range of needs, including:
- Adjusting to or managing long-term health conditions that aren’t fully addressed through medical treatment
- Mental health and emotional wellbeing
- Relationship challenges
- Domestic abuse
- Loneliness and isolation
- Low self-esteem or confidence
- Inadequate or unsafe housing
- Financial difficulties, including benefits
- Support for carers
- Coping with grief or bereavement
- Achieving health-related goals, such as managing weight, improving diet, quitting smoking, or increasing physical activity
How can they support me?
Social Prescribing Link Workers will:
- help individuals to identify and focus on what matters most to them and the factors impacting their wellbeing
- empower individuals to take greater control over their health
- connect individuals with local groups, activities, and community resources
How do I book an appointment?
To book an appointment with a Social Prescribing Link Worker, speak to your GP, or call the reception team on 01392 255722 who can make a referral on your behalf.
