The doctors welcome new patients who live within our practice area. The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them. You can download the guide below.
As it often takes some time for records to be forwarded from the former practice, all newly registered patients will be asked to complete the below questionnaire which includes questions around patient health.
If wanting to register at Mount Pleasant Health Centre you will need to complete the appropriate registration questionnaire. We also normally request proof of identification (i.e Passport or Driving Licence) and proof of address but due to the ongoing pandemic will be able to register you without these. We may at a later date request you bring these in to us.
Please fill out as much of the appropriate questionnaire as you can. Many parts are compulsory.
Completed forms are processed each week. We will aim to have you registered on the system by the first Sunday after you have submitted the form.
Medical treatment is available from the date of registration (the date the questionnaire is processed by the administrative team). Please contact reception for further information.
