Children and Young People’s Social Prescriber, [PCN role]

Details

This is an exciting new role for a Social Prescriber with a special interest in the wellbeing of children and young people to join the Nexus Primary Care Network. The post holder will work equally across the five network practices but will be based at Mount Pleasant Health Centre in Exeter.

This role is primarily to work with children and young people aged up to 25, but may also involve work with their parents/carers. Social prescribing empowers people to improve their health and wellbeing through referral to a non-medical link worker who gives time, focuses on what matters to the individual and takes a holistic approach, connecting people to community groups and statutory services for practical and emotional support.

Pay – £23,000 to £26,000 a year     Hours – Full time (37.5hrs/wk) or Part time (to be negotiated)

Mental Health and Wellbeing Care Coordinator, [PCN role]

Details

This is a new role working within the Nexus Primary Care Network (PCN) Mental Health and Wellbeing Team in delivering a contemporary, multi-disciplined Primary Care Wellbeing Service that aligns with and supports the CMHT and wider system. The role will have the confidence to work across Secondary, Primary and Community services.

The post holder will deliver high quality wellbeing services. The role involves managing a caseload to meet the health and wellbeing needs of the patients.

Pay – Negotiable   Hours – Full time (37.5hrs/wk)

Advanced Nurse Practitioner, [GP Practice, Clinical Role]

Details

The post holder will work as an autonomous independent nurse practitioner as part of the urgent care duty team.

They will be required to promote and provide nurse practitioner services in the treatment of patients presenting with acute complaints and with specialist knowledge and skills, ensure the delivery of quality patient care.

Duties may include minor injury work alongside other urgent care.

You will be a qualified and registered advance nurse practitioner and independent prescriber, with excellent people skills. Experience of effective working as part of a multidisciplinary team is essential.

Pay – Negotiable   Hours – Part time

PMA Kickstarter Placement – [GP practice, assistant admin role]

Details

The PMA (Practice Managers Association) have been appointed by the Department of Work and Pensions (DWP) as a Gateway provider for the Kickstart scheme UK-wide initiative (excluding Northern Ireland). The Kickstart Scheme is for those aged between 16 and 24, who are currently claiming Universal Credit, and may be at risk of long-term unemployment. The 6-month placement will support you in developing the skills and experience you need to find work after completing the scheme.

This role will see you develop administrative skills in a primary care setting. Supporting the vaccine rollout, the wider clinical and non-clinical teams and the Primary Care Network as a whole.

Pay – National Living Wage      Hours – 25 hours per week
